Your First Visit

Your first visit is the first step to a fantastic smile. Please bring your completed new patient forms to your appointment. When you arrive, our front desk will review the forms and discuss the reasons for your visit. In the absence of gum disease, you’ll receive a dental cleaning, and we’ll take a set of X-rays. Then you’ll meet the dentist for a comprehensive examination, including:

  • A tooth-by-tooth check for decay
  • An oral cancer screening
  • An assessment of periodontal (gum) health
  • A bite evaluation

The dentist will discuss your current oral health status, as well as your short and long-term dental goals. Then, he’ll explain his treatment recommendations and answer your questions. We make it a point to provide patients with all of the information they need to make decisions that meet their objectives, fit their budget, and work with their schedule.

New Patient Forms

For added convenience, we’ve now made it possible for new patients to download and print all of the paperwork they’ll need to complete below. Just remember to bring the pages with you to your first appointment, and our front desk team will be able to expedite your check-in process. Alternatively, we can mail/fax the forms to you upon request, or you can complete them in our office at the start of your initial visit, although we do recommend that you arrive a little early in that case.

Dental Insurance Accepted

Rowlett Dental Associates is in-network with Delta Dental, and Dr. Pollock also accepts several in-network PPO insurances for patients under the age of 18. We are always willing to provide services as an out-of-network provider and will file claims on the patients’ behalf so that they can maximize available benefits without having to lift a finger themselves. Additional payment options include cash, check, American Express, Discover, MasterCard, and Visa.

Do you and your family not currently have dental insurance? Are you interested in an extensive treatment plan but cannot cover the entire cost upfront? No problem – affordable financing can be arranged through CareCredit.
